1. WHEN IS TOOTH WHITENING INDICATED?

🚫 Contraindications / cases requiring caution

  • Severe intrinsic discoloration (tetracycline staining, severe fluorosis)
  • Untreated caries, cervical abrasion, cracked enamel (must be treated first)
  • Pregnant or breastfeeding women (treatment should be postponed)
  • Patients with severe, uncontrolled tooth sensitivity

2. WHY DOES TOOTH WHITENING CAUSE TOOTH SENSITIVITY? 

2.1. Mechanism of sensitivity during whitening

Professional in-office whitening often uses high-concentration Hydrogen Peroxide
(e.g. Pola Office: 35–37.5% Hydrogen Peroxide)

  • Small molecules → rapidly penetrate enamel & dentin
  • Reach dentinal tubules → stimulate fluid movement
  • Activate odontoblast receptors and A-delta nerve fibers

👉 Sensitivity occurs during or after whitening due to:

  • Temporary opening of dentinal tubules
    Free oxygen increases enamel permeability (reversible), especially in:

    • Cervical areas
    • Teeth with recession, abrasion, thin enamel
  • Hydrodynamic theory (Brännström)
    Oxygen release, heat, and surface dehydration cause dentinal fluid movement → sharp pain
  • Patient-related factors
    History of sensitivity, thin enamel, orthodontic treatment, enamel reduction, stress or anxiety (lower pain threshold)

2.2. Solutions to minimize sensitivity effectively

A. Before whitening (MOST IMPORTANT)

  • Careful screening
    Do NOT whiten if there is gingivitis, severe recession, exposed dentin, untreated caries, or enamel cracks
  • Pre-treatment desensitization (7–14 days prior)
    Desensitizing agents:

    • Potassium Nitrate
    • Arginine
    • CPP-ACP
      👉 Instructions: Apply directly on teeth for 5–10 minutes/day

B. During whitening

  • Avoid prolonged exposure
    • Divide into 2–3 short cycles instead of one long cycle
  • Reduce thermal stimulation
    • Do not place the light too close
    • Avoid high-power light for extended periods
  • Protect cervical areas carefully
    • Apply thick gingival barrier
    • Optional: apply desensitizer before whitening gel

C. After whitening (MANDATORY)

  • Apply desensitizing agents immediately
    • Potassium Nitrate
    • Fluoride gel
    • ACP / CPP-ACP
  • Avoid hot, cold, acidic foods and beverages for 48 hours
    • No hot or cold foods
    • No acidic foods, alcohol
    • No smoking
  • Pain relief if needed: Paracetamol (for severe sensitivity)

2.3. Clinical experience

  • Sensitivity usually peaks within the first 24 hours
  • Resolves after 48–72 hours
  • Rarely lasts longer than 5 days
  • Does NOT damage the dental pulp when properly indicated

2.4. Conclusion

“Tooth sensitivity after whitening is a temporary physiological response caused by peroxide penetration through enamel and dentin.
It does NOT mean tooth damage or pulp necrosis and can be completely controlled with proper preparation and aftercare.”
